Completed application form (downloadable)

Copy of the child's original birth certificate or in the absence of this, any other document that can support the fact that she/he is your child

 

Copy of the child’s health insurance card/ booklet

Address card or identity card (if the child is not of Hungarian nationality)

 

Registration fee has to be paid
